Library Card Registration

Required items for adults

• Full name including middle name or initial
• Signature
• Valid photo ID
• Proof of current address
• Date of birth

Required items for minors (under age 18)

• Full name including middle name or initial
• Signature of parent/guardian
• Proof of current address
• Date of birth
• Must be at least 5 years old

Cardholder responsibility

• Parents/guardians are responsible for items checked out by minors.
• Cardholder must be present to check out items on his/her card.
• Cardholder may obtain a replacement card after a one week waiting period from the time the card is reported lost.
• Cardholders will be charged $5.00 for a replacement card.
• Stolen cards will be replaced free of charge, if the cardholder provides a copy of the police report.
• It is the responsibility of the cardholder to notify the library of any change of name, address, etc.

Inter-library loans

Most material not available at the Delta Public Library can be requested from other libraries, usually at no charge. A special request form must be filled out and turned in at the circulation desk. A fee of $1.50 PER TITLE will be charged to the patron if materials are not picked up within 7 days following notification, to help defer the cost of delivery. Inter-library loan materials carry a 25 cent per day fine.

Exterior book drop

An exterior book drop is located on the east side of the building (along the drive between the library and the bank) and is open 24 hours a day for your convenience.

Equipment

An overhead projector, and portable screen may be borrowed from the library. Equipment is loaned for a three day period. There is a $5.00 deposit required on all equipment. These deposits are refundable upon return of the equipment in good working condition.

Red Carpet Service

Residents of the Delta School District who are unable to use the library because of physical disability, extended illness, or lack of transportation are eligible for our homebound delivery service. Please call the library at 419-822-3110 for further information.
